By my signature below, I recognize that as a collaborator at the University of Vermont (UVM), I have access to student registrations through the Maxient Conduct Manager database. I understand that the information in this database is strictly confidential. I am familiar with the federal (studentprivacy.ed.gov/) and university policies (www.uvm.edu/policies/general_html/privacy.pdf, www.uvm.edu/policies/student/ferpa.pdf) that govern my use of these files. I also understand the consequences – personal and institutional – of the mismanagement of these files. I understand that a review of my use of the Maxient database may reveal that I have searched or edited information that has nothing to do with my work or outside the area of my access. If this is the case or if I cannot protect student information, I will be subject to discipline and I may lose access to the Maxient database. I understand that I must confidentially and securely protect all personally identifiable information about UVM students and that I am only allowed to access information as long as I have to do my work on a case that is verified by the relevant department within the studentaffairs department.
